NOTE:

Patient is 71 years old Hispanic male living in the long term care for over 3years. Resident is alert oriented x 3. Patient was admitted with Major depressive disorder and schizophrenia. Patient is on Dupropion 20mg 1 tab PO daily. Patient was seen by psych MD and gave an order to increase Dupropion to 25mg daily. Paitent is stable at this time. He was encourage to report any reaction.
