basic html css code for a software interface design for a simple application 1

Hi I need help with designing a software interface for a simple application interface. THIS PART IS ONLY FOR THE USER INTERFACE DESIGN USING HTML AND CSS. Although you will need to document the flow of the application from screen to screen, the request for this assignment right now is just the sample interface. Either way, I will need the HTML and CSS code for the assignment. The interface should follow the same design as listed below. Please see the instructions below:

This is a meant for a mobile platform, so when designing the HTML and CSS please make sure it is for mobile devices, this will be utilized for a mobile application going forward. I have included a drawn image of the layout of the page I need the HTML and CSS code for. I have also included the image files to utilize. The header image, which should appear at the top of the page, is labeled “First_Coast_AACN”. The background image I would like to use the attached file labeled:”Healthcare2“.
